Transcript Request

(Please note this is for Continuing Education –POST DOCTORATE)  Should you require your undergraduate transcript, please contact the Registrar’s Office at 1-800-234-6922 or 1-315-568-3000.

Unofficial transcripts are made available after college sponsored continuing education seminars and most co-sponsored seminars.  These transcripts are provided at no charge, however, should you require an additional continuing education transcript, there is a $5.00 charge for an unofficial transcript.

If your state requires you to have an official transcript, (one with the director’s signature and the college’s raised seal), this will be mailed to you or to your board. (see below)  If you wish an official transcript for yourself, your insurance company, or another college, there is a $10.00 charge.

In most cases, attendees are responsible for maintaining their own records and reporting their hours to their individual state board when requested.  Some State Boards require that an official transcript or an electronic notification be sent directly to them; NYCC then complies with that state’s requirement.

Please call the PG Dept at 1-800-434-3955 or 516-796-5923 selecting option 3 or extension 133.  Leave a message and someone will promptly return your phone call.  Should you wish to request the transcript via e-mail, request by clicking here.  Someone will promptly contact you either via e-mail or telephone.  For either notification, be prepared to provide name, address and phone number.  If transcript requires payment, you will be asked to provide payment information: credit card number, (Visa, Mastercard, Discover), expiration date, three digit security code, cardholder’s name and billing address.

If you are requesting a transcript for a PT course taken from a college other than NYCC, but cosponsored by NYCC, please provide name of program, location and dates of attendance.  If you are unsure of this information, please be aware that the college will charge a Search Fee of $25.00.

Certificates are made available after college sponsored continuing education certificate programs and co-sponsored certificate programs.  These certificates are provided at no charge, however, should you require an additional or replacement certificate, there is a $15.00 charge.
